Das Ubermax-Gefängnis San Tiburon liegt versteckt im Großen Nordwesten. Zu den Bewohnern zählen Monster, Cyborgs und Superschurken, die alle mit Energiehemmern und Schockhalsbändern ausgestattet sind. Am berüchtigtsten unter ihnen ist The Lobe, ein Supergenie, das über ein unauffindbares Vermögen verfügt. Aufseher Delvin ist wohl genauso korrupt wie seine Schützlinge und sein einziges Interesse gilt den verborgenen Reichtümern des Lobe, die er seit Jahren zu knacken versucht. Der fragile Frieden des Gefängnisses wird durch die Ankunft von Payback, einem mörderischen Bürgerwehrmann mit bösen Absichten gegen das gesamte Gefängnis, und Diego Doval, einem Fahrer, der eine erfundene Strafe verbüßt, ins Chaos gestürzt. Während die Spannungen unter den Insassen und dem Personal zunehmen, herrscht im Gefängnis Anarchie und die Ordnung wird auf den Kopf gestellt.
